Look at the population density maps of some states and then realize that since there is no state-run hospitals in the US that most of the rural folk are depending upon pharmacies that might be 100+ miles away.
Here is an Oregon density map . Some of those towns of 500 people depend on a Walmart for everything, and it can be hours away.
