Yeah the key here is the deranged moments need to be visual. A man climbing into the most disgusting toilet in the country and swimming around in it as an allegory for the withdrawal psychosis he's falling into and the extreme lengths he would go to or debase himself disgustingly just for another hit is the right way to go about visualising an incredibly deranged internal machination.
If the writers are as good as I think they are then they can come up with creative ways to translate what they've done in the game into the new medium. The question is really whether or not they can get a director with the talent and the creativity to match them.
There are very few directors out there that I'd trust with this.

Oh shit Disco Elysium but with Kaiji-level visual metaphor to represent a character's thoughts might not be so bad depending on how it's executed. Oh yeah it worked on Trainspotting too, I just remebered the toilet scene.
Brilliant scene. Disco Elysium could be full of this shit with the right writer and director. You could have Harry falling into all kinds of deranged hallucinations and popping out of them regularly.

that's true but it could always go the way of Sherlock as twitter has quickly concluded.

Cuno would translate to screen 1:1 though.
